If you are unable to complete these forms at home, please arrive 15 minutes early so that you may fill them out before your scheduled appointment. In addition, please remember to bring:

  • A referral form from your primary doctor if required by your insurance company

  • Insurance cards

  • A complete list of your medications including the dosage and instructions for use. If you prefer, you may bring the medication bottles to your appointment.

  • Any pertinent medical records that may be helpful for the doctor to review

  • Names and phone numbers of other doctors participating in your care.
